The Homeless Families Transition Collaborative (HFTC)
THE HOMELESS FAMILIES TRANSITION COLLABORATIVE (HFTC)

The HFTC receives Housing and Urban Development (HUD) funding through The CT Women’s Consortium. The CT Women's Consortium is the lead agency for this collaborative which includes the following community based agencies:

APT Foundation Vocational Services,

The HFTC offers homeless women and homeless families, most whom are female heads of households, a continuum of integrated supportive services and transitional housing opportunities. The program fosters the stabilization and empowerment of homeless women ad homeless families from the time they are living in an emergency shelter to the time they are prepared to live independently in an apartment of their own.
